Families often ask why private clinics cannot prescribe more than 28 to 30 days of stimulant medication in one go. Some have been told that the NHS can issue larger quantities, which understandably causes confusion. It is important to clarify that this is not a Sanctum rule. It is a requirement based on the national governance framework for Schedule 2 controlled drugs. This article explains the legal responsibilities, professional standards and the rationale for the 30 day limit.
Most stimulant ADHD medications fall under Schedule 2 of the Misuse of Drugs Regulations 2001. This classification carries one of the highest levels of control for medicines used in routine clinical practice.
Schedule 2 status requires prescribers to follow strict rules regarding writing standards, record keeping, monitoring and risk management. These safeguards exist to ensure safety, prevent misuse and maintain accurate oversight.
Although the legislation does not specify a fixed number of days for each supply, national policy has established that Schedule 2 medicines should usually be prescribed for no more than 30 days. Any prescribing beyond this requires exceptional and clear clinical justification.
The 30 day recommendation comes from multiple national bodies and is set out in several formal documents. The NHS England Controlled Drugs Prescribing Policy states that Schedule 2, 3 and 4 controlled drugs should normally be issued for no more than 30 days. This policy is designed to reduce diversion, ensure regular monitoring and maintain safe governance.

Although some NHS services may appear to issue larger quantities, they operate within NHS systems that private providers do not have access to. Private prescribers must follow the national standard to ensure safety and legal compliance.
Private clinics do not have access to NHS controlled drug support structures. These include NHS pharmacy governance frameworks, Accountable Officers and centrally managed controlled drug audits. Because these systems are not available to private services, the full responsibility for safe prescribing rests with the private prescriber.
Regulators and medical insurers therefore expect private prescribers to keep Schedule 2 quantities within 28 to 30 days unless there is a clear clinical justification. Prescribing larger quantities increases risk, liability and the potential for misuse and would need to be documented in detail.

There is no formal approval process by NICE for ADHD assessments. However, our assessments adhere to and surpass NICE guidelines (NICE guideline 87) for ADHD diagnosis and management. Our comprehensive process involves gathering information from parents, family members, and school. We use objective data led testing. Our detailed assessments, typically lasting 2-3 hours, cover all aspects of the patient’s personal, medical, psychiatric, and family history.
ADHD medications require specialist initiation. GPs may not feel confident starting these medications without a Shared Care Agreement, typically arranged with local NHS services. Some GPs may accept shared care requests from private providers; check with your GP beforehand.
We use partner pharmacies to ensure safe and timely medication dispensing and delivery or we can post the prescription to you. Controlled drugs require specific paper prescriptions (FP10PCD) and can take up to 5 working days for delivery.
It usually takes a minimum of 6 months to reach the optimal dose. If multiple medications are trialled, this period may be extended. Once stable, we can request your GP to continue the medication under a Shared Care Agreement, with ongoing reviews every 6-12 months.
A medication titration plan costs £199 per month. Any additional reviews outside of scheduled plans, will incur costs.
